How to Refer

For individual pupils:

There is a referral form, a review request form and follow-up form in the downloads window on the right side of this page (pdf). Forms for pupil assessments and reviews are also available from the IAS and are only available to schools.

To ensure equality of opportunity and a staged process of assessment we ask that:

  • Pupils in KS1 are not usually referred as young pupils vary enormously in their rate of literacy learning. They need to be allowed time to develop oral and early written competencies
  • The school has systematically tried various strategies to support the pupil with literacy and progress still has not been made. This includes Wave 2 interventions and other additional literacy support. Please provide evidence when referring
  • Pupils have reached School Action Plus on the Code of Practice before the school requests advice from the Advisory Teachers
  • Schools prioritise pupils, and schools who have a trained member of staff should assess and support pupils using their own expertise
  • Priority is given to schools in high support categories
  • The school has discussed the referral fully with the Parent/Carer and permission has been signed for
  • The school is willing to meet to discuss the implementation of our advice

For whole school support, schools should contact the Advisory Teachers to discuss the support required.
